STUNT LESSON REQUEST FORM

PLEASE REFERENCE THE CURRENT SCHEDULE OF AVAILABILITY THROUGH YOUR iCLASS PARENT PORTAL PRIOR TO SUBMITTING THIS FORM SO YOU KNOW WHAT IS POSSIBLY AVAILABLE AS A STUNT LESSON.
TWO Instructors must be available at the same date/time in order for a stunt lesson to be booked.
This form is to be submitted if you are looking to book a one-time stunt lesson for your athlete.
If you are looking to book multiple stunt lessons, you will submit a separate request for each lesson.
Please allow 24-48 business hours from the date of submission for a response about your request.
Please be as thorough as possible with your request or you will receive a response that it cannot be fulfilled.
